ABOUT PATCH TESTING

DAY 1- Monday: Applying the patches.

Please take a shower before you come to the Office! It takes one week to complete the test and you cannot wash your back until Friday. Immediately after we apply the stickers,  you will have to sit in the office for 15-20 minutes to ensure that you are safe and do not develop anaphylaxis. You may take your usual anti-histamines. You may not be on Prednisone. 

Day 3- Wednesday, Detecting Acute Hypersensitivity

On Day 2, Wednesday, we will remove the patches. Then you will need to sit for 15-30 minutes before we can read the patches properly because the back tends to get wrinkled with the  sticky tape and is difficult to read. It "unwrinkles" in about 15-30 minutes-hence the wait. This is the acute hypersensitivity reading. This means if you develop a rash within a day or two of exposure, it will be positive and quickly. We will give handouts explaining the allergen, which are also available on true test.com. 

Day 5 Friday, Detecting Delayed Hypersensitivity

Please do not exercise or wash your back until Friday when we will do the delayed hypersensitivity reading. This means you may not develop a rash until after 96 hours of exposure. 

Some people have delayed hypersensitivity beyond 96 hours, many times it is days later! Please let us know if this occurs so we can fit you in, see you and document the delayed hypersensitivity. This is very important. If you just tell us "by the way" and later you need other treatments or medications to manage the delayed hypersensivity, your insurance may not cover it. Remember, if we don't document it, it did not happen.  Your insurance company relies upon our documentation to approve procedures, medications and treatments.  We will give handouts explaining the allergen, which are available in true test.com.

A "Patch Test" is designed to detect a skin allergy to over 80-90% of the most common allergens. Here at PDC, we use the three panel,  true test patch test. You are skin on the inside and skin on the outside!  Therefore, sometimes what we find can affect your gut. We may suggest dietary changes that can help your skin inside and out. We will also refer you to a Gastroenterologist.

This a Wednesday reading showing a severe, acute reaction to the allergen "Balsam of Peru", which is found in many products. This allergen was affecting the patient's GI Tract leading to pain, nausea and diarrhea because Balsam of Peru cross reacts with Vanilla, Cinnamon, Cloves and Nutmeg. It also reacts with Citrus Peels found commonly in spice mixes like Lemon Pepper.  See FAQ's for more

This is a Friday reading showing emerging reactions. Look at the bottom left square  (Fragrance Mix) that is positive on Wednesday. Above it is another reaction emerging to Caine Mix. This took 96 hours to be visible. This delayed reaction is the reason it is difficult to identify allergens without Patch testing. Notice the top left emerging allergies not really visible yet.

This was about a month after Patch testing and the patient was still showing positive delayed reactions. The top left allergens are fully visible now. These are to wool alcohol and colophony. This shows how critical Patch testing is to identify delayed reactions.  If you develop a delayed reaction like this patient please call us so we can see you and document it in your chart.

A patch test involves the application of three distinct patches across your back to account for about 80-90% of the most common skin allergens. You are skin on the inside and outside!  A Patch test may be able to identify some oral allergens associated with oral allergy syndrome. Balsam of Peru allergy is the best example of this. 


The patient must wait 15-20 minutes to avoid risk of anaphylaxis. They may leave the office after 15-20 minutes. 


Please come in freshly washed! This is crucial because you will not be allowed to wash your back for one week. If you take anti-histamines, you may continue. Most websites say to stop but after twenty years of experience, we know it really does not matter. It won't blunt your reaction significantly enough if you are truly allergic. It does help block any false positives, so we prefer you to continue the anti-histamine if you've been taking it regularly. 


You may exercise Monday morning then  shower before we apply the patches later that day. You may exercise Friday afternoon  after the final patch test reading. To ensure the most accurate patch test reading possible, please do NOT exercise or wash your back until after we do the delayed hypersensitivity reading on Friday.

There will be a copay and it will be billed to insurance. Bring any images you took so we can incorporate them into our notes.
